The completed proposal must include all requested materials and should be sent:

• by E-mail and be received prior to April 15th
• by Mail and be received prior to May 1st

Electronic submissions must be confirmed by the submission of the completed form in hard-copy with the nominator’s signature and must arrive by the ultimate deadline, May 1st, 2006.
